Examination revealed bilateral proximal interphalangeal joint synovitis and severe deformity and resorption of the distal interphalangeal joints with dystrophic nail changes and diffuse dactylitis. The fleshy growth was indicative of a sarcoid-specific “lupus pernio” presentation, confirmed by skin biopsy, showing noncaseating granulomas.
