Abstract
Author's note. The author was a recipient of a research career development award and was physician-investigator for 16 years, program director for 40 years, division chief and medicine department chair for 35 years, medical editor for 14 years, president of national organizations, and in academic medicine for ~55 years, with many of these responsibilities and activities overlapping. The following are his reflections on science (and medicine) today from the perspective of this background.
