Several sovereign small island developing states in the English-speaking Caribbean have a significant burden of inflammatory rheumatic diseases and many unmet needs in their overburdened government-funded healthcare systems. There are few rheumatologists, and with no rheumatology fellowship training programs or rheumatology divisions in any of these countries, rheumatologists tend to work alone with minimal support (Figure).
