Bilateral joint replacement of hip and knee joints in patients with rheumatoid arthritis

Summary

Between 1973 and 1981, bilateral total replacement of hip and knee joints was performed in 22 patients with rheumatoid arthritis. During follow-up, 2 patients died of diseases not directly related to the operation. Besides these patients, there were 2 patients who could not be followed up. The remaining 18 patients constituted the subjects for study. We studied the postoperative results, emphasing improvement in the ability to perform everyday activities. Walking ability was improved in 16 patients. There was an increase in the number of patients able to rise from a chair, go up and down stairs, and get in and out of a car, bus or train. Of the 14 married patients (not including 2 patients who were unable to walk and 5 patients over 60 years of age) 7 were able to perform sexual intercourse as well as they had been able to when healthy. The patients who underwent bilateral total replacement of hip and knee j oints often presented various problems, for example with regard to indications, the operative technique to use, as well as complications such as heart disease, pulmonary disease, and secondary amyloidosis. We have found, however, that bilateral total replacement of hip and knee joints can accomplish the operative objectives of eliminating pain and improving the quality of daily life in severely handicapped rheumatoid patients.

Zusammenfassung

Zwischen 1973 und 1981 wurde ein totaler beidseitiger prothetischer Ersatz von Hüftund Kniegelenken bei 22 Patienten mit Rheumatoidarthritis durchgeführt. Während des Beobachtungszeitraumes starben zwei Patienten, jedoch nicht an Folgen der Operation. Bei weiteren zwei Patienten war eine nachfolgende Beobachtung nicht möglich. Die restlichen 18 Patienten stellen das eigentliche Untersuchungsmaterial dar. Wir untersuchten die postoperativen Ergebnisse mit besonderer Betonung der Aktivitätsverbesserung im täglichen Leben. Mit Ausnahme von zwei Fällen war bei allen Patienten die Gehfähigkeit verbessert. Die Patienten konnten allein vom Stuhl aufstehen, Treppen hinauf-und hinuntersteigen und in Autos, Busse und Züge in zunehmenden Maße ein- und aussteigen. Den Geschlechtsverskehr konnten sieben verheiratete Patienten genauso wie in der Zeit vor ihrer Erkrankunf ausführen(von sieben weiteren der insgesamt 14 verheirateten Patienten waren zwei gehunfähig und fünf älter als 60 Jahre). Bei Patienten, bei denen der totale beidseitige prothetische Ersatz von Hüft- und Kniegelenken durchgeführt wurde, gab es öfter verschiedene Probleme, z.B. im Hinblick auf die Indikation, die Operationstechnik, Komplikationen durch Herz- und Lungenerkrankungen sowie durch sekundäre Amyloidose. Jedoch ist lestzustellen, daß durch den totalen beidsietegen prothetischen Ersatz von Hüftund Kniegelenken eine Qualitätssteigerung des täglichen Lebens bei sonst schwer durch Rheumatoid-arthritis beeinträchtigten Patienten erreicht wird.
